Details: 
Onsite near Fremont, CA
Pay Range: $52+/hour (depending on experience) 

  • ESY 2024 and Full 2024-2025 school year
  • ESY Dates: 6/6/24-7/3/24, 20-30ish hours/week
  • 8/12/24-6/2/25, full time, 40 hours/week
  • K-12th grade caseload 
  • District wide position working in collaboration with 2-3 other PTs
  • Multiple school sites (mileage can be reimbursed between school sites) 
  • Very supportive district

Responsibilities:

  • Conduct comprehensive evaluations of students to assess their physical abilities and limitations.
  • Develop individualized treatment plans based on evaluation results and collaborate with the school team to integrate therapy into the student's educational program.
  • Provide therapy services to students in individual and group settings.
  • Monitor and document progress, modify treatment plans as necessary, and communicate regularly with the school team and parents/guardians.
  • Provide consultation and training to teachers and other school staff to support the integration of therapy goals into the classroom and daily routines.
  • Participate in IEP meetings and assist in the development of appropriate goals and objectives.
  • Maintain accurate and complete records, including progress notes, reports, and billing documentation.

Requirements:

  • Graduate degree in Physical Therapy from an accredited program.
  • Valid California Physical Therapy license and certification from the National Board for Certification
  • Must be able to pass a background check
